GNPTG-mucolipidosis

GNPTG-mucolipidosis is a very rare lysosomal disorder most often observed in the Middle East. It is caused by alterations in the GNPTG gene, which plays a role in lysosomal enzyme targeting. Affected individuals typically show symptoms in early childhood, including a slowing of the growth rate and joint issues. The exact prevalence is unknown, and this condition remains an uncommon diagnosis.

Individuals with GNPTG-mucolipidosis almost invariably experience musculoskeletal abnormalities. Key features include finger joint contractures, joint stiffness, shoulder contractures, and claw hand deformities, accompanied by skeletal differences such as a flat capital femoral epiphysis, hyperlordosis, genu valgum, pes planus, and a short neck. Myopia is consistently observed, and many affected individuals also exhibit short stature. Additionally, there may be a gradual, mild coarsening of facial features, while cognitive function remains normal or only slightly impaired. Not all individuals experience all features, and severity varies considerably.

GNPTG-mucolipidosis results from pathogenic variants in the GNPTG gene. The GNPTG gene encodes a component of the enzyme complex responsible for targeting enzymes to the lysosome, and disruptions in this process lead to the accumulation of substrates and subsequent cellular dysfunction. This condition is inherited in an autosomal recessive pattern, meaning that an individual must inherit two altered copies of the gene—one from each parent—to be affected. Genetic counseling is recommended to help families understand recurrence risks and options for family planning.

Currently, management focuses on supportive care and symptom management, with treatment tailored to each individual’s needs. A multidisciplinary team approach can help address joint stiffness, pain, and other emerging symptoms through interventions such as physical therapy, orthopedic care, and regular monitoring.
