Smith-Magenis syndrome

Smith-Magenis syndrome is a multisystem genetic condition that affects neurological development, sleep regulation, growth, and facial skeletal formation. It results from changes in the RAI1 gene, which plays an important role in regulating other genes during early development. Most individuals are identified in infancy or childhood when developmental delays and distinctive facial features become apparent. Smith-Magenis syndrome is caused by pathogenic variants in the RAI1 gene. RAI1 helps control the activity of multiple other genes involved in brain development and circadian rhythm, so changes in RAI1 disrupt several body systems. The condition follows an autosomal dominant inheritance pattern, meaning one altered copy of the gene can cause symptoms. However, most affected individuals have the variant for the first time in the family (sporadic), so their parents are typically unaffected. An individual who has Smith-Magenis syndrome has a 50 percent chance of passing the altered gene to each child. Because the presentation differs widely (variable expressivity), family members who inherit the same variant could be more mildly or more severely affected. Regular surveillance is an essential part of long-term care, helping to identify potential complications before they become serious. Suggested monitoring typically includes periodic eye examinations for strabismus, assessment of growth and nutritional status to manage obesity, and evaluation of sleep patterns and behavioral health. Families should discuss individualized surveillance schedules with their healthcare team. Smith-Magenis syndrome is a lifelong condition, but life expectancy is thought to be near normal when medical complications are well managed. Severity and symptoms vary widely between individuals, and early access to therapies, educational accommodations, and behavioral interventions can make a meaningful difference.
