X-linked reticulate pigmentary disorder

X-linked reticulate pigmentary disorder is an extremely rare skin condition that has been described in only four families. In males, the condition is marked by widespread, net-like brown skin changes that start in early childhood, along with a number of systemic issues such as lung infections, eye problems, and gastrointestinal inflammation. In females, the skin changes are more localized and follow the natural skin patterns, known as the lines of Blaschko. This condition was once thought to be a form of cutaneous amyloidosis, although the presence of amyloid deposits is not always consistent. The disorder is linked to genetic factors and shows different features depending on the patient’s sex. In males, the condition presents with widespread, net-like brown skin lesions that begin in early childhood. They may also experience recurrent pneumonia, corneal opacification (cloudy areas in the eyes), inflammation in the gastrointestinal tract, urethral narrowing, poor growth, reduced sweating, digital clubbing (changes in the fingers), and unusual hair features such as unruly hair and flared eyebrows. Females typically show only localized brown hyperpigmented skin lesions following the natural lines of the skin.

X-linked reticulate pigmentary disorder is linked to variations in the POLA1 gene. This gene change contributes to the condition, and the pattern of inheritance is X-linked. This means that the genetic alteration mainly affects males with more widespread symptoms, while females generally have milder, localized skin findings.
